By completing this form within 12 months of your purchase correctly with accurate personal information and details of when and where purchased, you are entitled to an extended warranty of 3 years in total from date of purchase. The warranty covers faults not caused by abuse or neglect and includes parts and labour but does not cover shipping and related costs.
